You don’t insta-die in Exile if you don’t have food. You take a small amount of damage when you should eat, i think you might temporarily weaken in other ways as well. But the main way to heal in that game is resting on the overworld, and you can’t do that without food.
Regarding that puzzle, I think there is a note, or two notes in pieces, that you find that has the combination on it. They might not be in the same dungeon as the gate, I don’t remember for sure. I can look it up for you if you’re stuck and don’t want to have other things spoiled for you looking at an FAQ, but I don’t remember off the top of my head.
Speaking of games where you die for running out of food and are punished mercilessly for missing stuff, this review dropped today:

That looks like the one of the most frustrating games of all time. If you don’t want to watch the entire video, the part I was referring to starts at 1:40.
That Sisters of Mercy video looks like it repurposes the set of a movie I’d totally watch, and probably have.
The Cuntry Disco song should be used in a parody of that scene in Say Anything (I’ve never actually seen the rest of that movie).
